Do you have enough life insurance? Open enrollment for MetLife’s Supplemental Life Insurance is now open through December 20, 2019!

Supplemental life insurance is a voluntary plan and it is a deduction on your payroll. If you are interested in learning more about the MetLife’s Supplemental Life Insurance, please review the attached flyers pertaining to the coverage.

If you are interested in enrolling you would complete the enrollment form and forward the form to Dana Holman, Benefits Coordinator at District Center. The original form must be sent and received in District Center no later than December 20th, 2019.

SPECIAL NOTES******YOU MUST ELECT COVERAGE FOR YOURSELF IN ORDER TO COVER A SPOUSE OR A DEPENDENT CHILD

  • Coverage for the employee can be obtained for an amount that reflects the lessor of 3X’s your salary or $150,000 without a statement of health and up to a maximum of the lessor of 5x’s your salary up to $500,000 with a statement of health.
  • Coverage for your spouse can be obtained for an amount equal to $25,000 without a statement of health up to a maximum equal to the lessor of 50% of your supplemental request or $150,000 with a statement of health
  • Coverage for your dependent child can be obtained for an amount equal to $10,000 or less up to the age of 25 with no statement of health required.

SUPPLEMENTAL LIFE INSURANCE IS OPTIONAL! SIGNING UP IS NOT AN OBLIGATION, THERE IS A PAYROLL DEDUCTION FOR THIS ELECTION.

ARE YOU ALREADY ENROLLED AND YOU WANT TO MAKE CHANGES TO YOUR PLAN?
If you are already enrolled on the supplemental life insurance plan and you want to increase/decrease your coverage amounts or add spouse/dependent coverage to your plan, you may make these changes at this time. Please complete a new enrollment form, this form will supersede the form on file.
