If your student(s) will need to attend a daycare facility or a home daycare provider, they must be in accordance with the Five-Mile Policy. Once you have selected a daycare facility or provider that meets the criteria of this policy, please complete the Daycare Transportation Request Form and submit it to your student’s teacher.
